WAIVER OF LIABILITY AND ASSUMPTION OF RISK

By submitting this application the registrant undersigned, hereby agrees to waive any and all liability and indemnify the Kansas City Area Pyrotechnicians (KCAP) and each of its members, directors, officers and agents from and against all claims, injuries, or damages direct or consequential from any level of participation in activities associated with Kansas City Area Pyrotechnicians. This would include but is not limited to: displays, demonstrations, seminars, training, transportation of fireworks and educational materials. The undersigned further agrees to assume liability and supervise their minor children at all times.

I understand that if I am accepted as a member, any false statements, omissions, or other misrepresentations made by me on this application may result in my immediate dismissal. I hereby state I have read the waiver, and will agree to strictly abide by the waiver, all bylaws, and safety rules of the Kansas City Area Pyrotechinicians. Violations of the above said bylaws and rules is cause for immediate suspension of activities and privileges,
which may result in expulsion.
